 MISSION

JDJ Transitions mission is to be instrumental in the transitioning of formerly incarcerated men and women (program participants). Providing assistance and support in facing challenges and barriers that are involved when re-entering communities and families.

 

 VISION

To assist and educate program participants to navigate the transition process.

GOALS

  • Provide services that focus on mental health support for the program participants and families
  • Implement evidence-based programs that target crime-producing behavior
  • Focus on changing criminal thinking and reducing criminal behavior
  • Build individual capacity of the program participants to be a productive member of his/her family and community
  • Link program participants to program services necessary for successful transition and reentry into the community
  • Increase the community capacity to address the offender’s needs and identify community resources to match assessed needs
  • Promote public safety through collaborative state agency and community partnerships that support offender transition to the community thereby reducing recidivism among the formerly incarcerated population

Linking Program Participants to Valuable Resources

Knowing about and accessing services available in a local community is a critical component of JDJ Transitions program. Staff members provide participants with access to a range of services:

  • health
  • family service
  • social service
  • residential treatment
  • mental health
  • domestic violence
  • legal advocacy
  • permanent supportive housing
  • entrepreneurship pathways
  • employment resources

     

To enhance program participants’ reentry success, community services include a resource library, on-site events and referrals from staff.

COMMUNITY CONNECTIONS

  • Resource Our reentry staff gathers information from community service providers to connect program participants with additional services that can continue even after they finish reentry programming. This information is available in a resource library maintained at each center.

  • On-site Events Community events are hosted on a regular basis, where representatives from local organizations or businesses inform participants about the resources available and provide motivation for engaging in these services. Participants have the chance to speak individually with representatives and to sign up for services, as applicable.

  • Referrals JDJ Transitions staff can contact identified service providers and initiate appointments for participants. Staff members follow up with service providers to confirm program participants attended their appointments and maintain open communication to help ensure seamless service delivery.

  • Local Outreach developing a network locally is essential to the success of the program and participants. Staff attend many local events and meetings with stakeholders and other service providers to expand their awareness of local resources accessible to program participants.

  • JDJTransitionsConnect expands through social media connections and referrals made by our program staff, participants can access JDJTransitionsConnect, through social media that includes a resource database to search for local resources that support basic needs, employment, and educational and vocational opportunities. The platform also includes a library of inspirational stories and videos.

Nathaniel Jones

Nathaniel Jones

President, CEO

Nathaniel Jones is a sixty-six-year-old established Peer Specialist, Human Behavior Associate, and Advocate who serves as an embodiment of vitality to others. Born in Memphis Tennessee, he attended the prestigious University of Wyoming, Tennessee, and Lawson State Community College. Seeking to widen his horizon of learning, he received 4000 hours of cognitive training. He is also trained in fatherhood and in living with others. 

Nathaniel currently serves at RLD Resources. In his career years, he has been on major front-lines of prominent organizations like Sports-Plus and Paradigm Security. He was formerly the CEO of Precise Menswear. Moreover, he harnesses hands-on problem-solving skills, a positive attitude, and an unwavering commitment to redefining and surpassing an organization’s set goals at his workplace. Nathaniel is most passionate about helping others as he reaches out in diversity to help enhance how you think. He also specializes in family unification.

Playing a vital role as an advocate, he is responsible for assisting in re-entry for those returning and attempting to renew and transition to help with the fight against recidivism. He harnesses cognitive-behavioral, preventive strategy, person-centered, motivational interviewing, psycho-education, and other modalities in providing individual and group counseling. Nathaniel also manages job placement with a ready-for-work program. Throughout his years of service, he built an outstanding reputation for himself.
